Recently a colleague who is a parachute instructor told me that the plane he jumps from crashed. As a pilot, that sent chills up my spine. I guess you don't have to be a pilot for it to concern you. Of course, I started asking questions. What happened? Was anyone hurt? How did it happen? My incident investigator brain kicked in and I started looking for root causes, lessons learned, so on and so forth. What I learned disturbed me.
